Use the dotphrase .housinghealth to help write an advocacy letter for your patients with poor housing conditions.
For any positive SDOH screeners or housing quality issues, please place a CHW referral. For inpatient needs, send a message to the "CHAM Community Health Workers" group on Epic chat. For outpatient needs, place an "Ambulatory Community Health" order and choose the corresponding dropdown option.
